In order to establish your new utility service(s), we must obtain some
information about you. Please enter the requested information below,
print, sign and fax to

along with a copy of your drivers license, lease or proof of ownership. Please provide us with a
day time contact phone number in order for us to contact you with the
service order number and to submit your payment for deposit of
$145.00 or $160.00. We know you have a busy schedule and if it's
more convenient please contact BTU Customer Service at 979-821-5700
to submit your payment over the phone once you receive
confirmation that your fax was successful.
